Chronic Health Conditions Chart
Chronic health conditions are displayed for each tract in Kent and Sussex Counties. The prevalence of each health condition is compared between each individual census tract and the state. The three conditions with the largest disparity (disparity being the increase between the state level and the census tract) are identified and color-coded, from smallest to largest using yellow to red. Percentages indicate the prevalence of condition among adults in the state or county.

Social Vulnerability Index
The Social Vulnerability Index (SVI) was created by the Centers for Disease Control and Prevention (CDC) for the purpose of identifying communities that may need support before, during, or after disasters. Socially vulnerable populations are identified using factors such as socioeconomic status, household composition, minority status, and housing type and transportation.
In the State of Delaware, Kent and Sussex Counties score particularly high within the SVI Index.
